작성 유의사항
1. "신청사항" 중 해당하는 세부 항목에 모두 체크
   (예: 한국학과 한국학전공 석사생이 TOPIK 3급 성적증명서를 제출하여 한국어 요건 충족 및 선수강과목 면제 희망 시,
          외국어성적제출(A) 및 선수강면제(B) 항목 체크)
2. 신청서 제출 전 하단에 서명 必
